HomeCare Angel: Remote Patient Monitoring

Homecare Angel is the designation of the first joint project resulting from EDGE's strategic partnership with the Hospital do Espírito Santo de Évora, in which EDGE's specialised know-how in Big Data platforms for intelligent ambient living and wellbeing support the Évora Hospital in the development of its remote patient monitoring system, to be functioning at the patients' home.
The need to follow the status and evolution of its patients' health condition, both in-between consults and after discharge, has led the Évora Hospital to explore the potential of the new mobile and digital technologies in the development of a remote patient monitoring system capable of offering the same level of excellence in healhcare delivery to the patients outside of the Hospital's facilities. Using a set of mobile health and medical devices, the Évora Hospital's remote patient monitoring system will allow the Hospital's patients to send their physiopsychological data and information on their ambient living to the healthcare team that is accompanying them. These professionals monitor continuously the received data that allows them to know the patients' clinical and wellbeing condition at all times. Whenever the system identifies deviation or deterioration of a patient's physical or psychological condition, an alert is triggered, enabling the medical professionals to be aware of the situation, to confirm it and to determine the best course of action, safeguarding the patient's wellbeing and quality of life.
Creating a direct connection between the Hospital and the patient, the Homecare Angel project aims to reduce the number of visits to the emergency service, as well as the number of hospital readmissions and the hospitalisation stay. At the same time, the project ambitions a lower incidence of hospital infections in treated patients a a higher success rate for health interventions, based on the early identification of complication and deterioration signs associated with the patients' conditions.
It is important to refer that the Évora Hospital's new remote patient monitoring system is being designed in close collaboration with EDGE, so as to observe nonobtrusive features and the full respect of the patients' privacy rights, abiding to the General Data Protection Regulation. Also cybsersecurity issues are being addressed with a clear goal to prevent cyber incidents that affect this new service and the remaining Hospital services and systems.
