eCare Reduces the Workload of Health Professionals

The eCare Platform is a smart personalised care intelligence platform that enables the remote and unobtrusive monitoring of health and wellbeing parameters at home.
Using the eCare Platform, health professionals may easily and efficiently monitor patients in their care who suffer from chronic diseases or are recovering from surgery. Intuitive dashboards and sophisticated visual tools allow medical teams to accompany the evolution of their patients’ health conditions, early identifying localised risks of relapse or rehospitalisation and receiving alerts immediately when their patients’ symptoms reach critical levels that worsen their overall health condition, justifying adjusting medication, changing treatment, making contact or speeding hospitalisation.
Through a friendly interface, health professionals may monitor patients’ vital signs (temperature, heart rate, blood pressure, blood glucose and oxygen saturation) and physiological parameters (weight, height, body mass index), health (medication, side effects, allergies, conditions, symptoms) and wellbeing (lifestyle, diet and nutrition, mood, sleep quality and physical activity) data. These parameters are collected either automatically or manually from medical and health devices and wearables. Also, the eCare App allows patients to fill-in simple, short questionnaires about their symptoms, medication adherence, sleep quality and quality of life. The eCare Platform’s intelligence capabilities optimises the patient monitoring functions already exercised by the health professionals, allowing to reduce their workload, while they easily identify the health status of each patient and provide each the personalised attention they require at any given time.
Aside from convenience, scalability and affordability, the adoption of the eCare Platform for remote patient monitoring brings significant benefits for health professionals and patients alike: it allows a real reduction of hospital bureaucracy and a better use of hospital resources (lower number of hospital admissions and visits to the emergency service), it facilitates the adoption of health preventative measures, it promotes patient independency and health literacy, it reinforces the completion of treatments and significantly boosts confidence and trust in the quality of care. As a result, the eCare Platform promotes better health outcomes and increases the overall quality of care delivery.
The eCare Platform will support the large-scale pilot campaign of the European Project SHAPES, which will occur throughout 24 months, involving 15 hospitals, clinics, nursing homes and home care in tem European countries and more than 2000 older persons. This effort will demonstrate clearly the superior capability of the eCare Platform to support and extend the autonomy and independency of the older population at home.
